使用人士個人資料收集聲明

1.Purpose of Collection 收集個人資料的目的 The personal data provided by you to Align Chiropractic Healthcare Centre (“Align”) will be collected, retained, processed, used and transferred for the following purposes: 康本脊骨神經醫務中心（以下簡稱為「康本」）將會收集、保存、處理、使用及轉移使用人士向理大提供的個人資料，用作以下用途： oProcessing your registration and appointment requests; 處理有關使用服務之登記及預約事宜； oDelivering services to you; 為使用人士提供服務； oVerifying your identity; 核對使用人士之身份； oHandling your complaints or enquiries; 處理使用人士提出之投訴或查詢; oFacilitating communications with you; 促進使用人士跟理大之間的溝通； oEnabling Align to comply with any applicable procedures, laws, regulations or court orders (in each case whether in Hong Kong or overseas), any requests by government, statutory, regulatory or law enforcement authority, and valid legal processes, ordinances obligations; and 協助康本根據任何適用程序、法例、法規或法庭命令（不論在香港或境外）遵守任何政府、法定、監管或執法機關的要求、有效法律程序及條例規定義務；及 oOther purposes directly relating to any of the above. 其他與上列任何一項有直接關係的用途。 2.You are required to provide your personal data, other than those items indicated as optional. Any data access and correction request according to the Ordinance should be made in writing to Align Chiropractic Healthcare Centre, Unit A, 8/F, Man Hing Commercial Building, 79-83 Queen’s Road Central, Central, Hong Kong or by email at contact@alignspinehk.com. 使用人士有權要求查閱及修正Align所持有的個人資料。根據條例規定，所有查閱及修正資料的要求應以書面形式提出，並致函至香港中灣皇后大道中79-83號， 萬興商業大廈8樓A室 康本脊骨神經醫務中心或電郵至contact@alignspinehk.com。 5.Security, Accuracy and Retention of your Personal Data 安全、準確及保留使用人士的個人資料 Align takes reasonable precautions to prevent the loss, misappropriation, unauthorized access or destruction of your data. Align also takes reasonable steps to ensure that all personal data held by it is accurate, complete, correct and reliable for the intended use. 康本採取合理的措施以防止使用人士的個人資料遺失、挪用、擅自使用或毁壞。康本採取合理的措施以確保保存之全部個人資料為準確、完整、正確及可靠以便作有可能之使用。
